In this dissertation work the antiarrhythmic efficiency of medicamental (finoptinum, allapininum, renitek, kozaar) and combined therapy in patients with chronical ischemic heart disease complicated by heart rhythm disturbances has been substantiated. The modern, adequate and high informatic methods of investigation were used (radioimmunological, spectrophotometric, biochemical, electrophysiological). The change of metabolism, neurohumoral systems regulation of homeostasis (SAS, FFA, PLO, AS, RAAS, KKS, function of paranephroses), hemodynamics and endocardic kinetics were determined. These change correlate to gravity and forms of cardiac rhythm disturbances, degree of treatment efficiency. They are estimated as pathogenetic factors of disease development. The criteria of prognosis and antiarrhythmic treatment efficiency, principles of differentiated and pathogenetic therapy of arrhythmia with using of membrane depressors, calcium antagonists, inhibitors of ACE, blockers of angiotensin-II receptors, combined applying of medicamental remidies and antioxidants, nonmedicine methods of treatment have been designed and scientificaly substantiated.
