The aim of investigation was to establish the sequence of the disturbances of diastolic and systolic ventricular function in the patients with arterial hypertension (AH) depending on the types of left ventricular (LV) remodeling, as well as possibilities of their regress during decrease of arterial pressure for more optimal evaluation of the effectiveness of long-term antihypertensive treatment. The study compared the parameters of the diastolic filling of heart ventricles in different types of LV geometry. It was proved that the status of cardiac diastolic filling depends not only on hypertrophy, but also on disturbances of LV geometry. The correlation and interlinks of left and right ventricular diastolic filling were studied. The determinants of right ventricular diastolic dysfunction in AH were established: thickness of the walls and the stage of LV hypertrophy. The stable normalization of the blood pressure during long-term adequate antihypertensive treatment, based on calcium antagonist amlodipine or angiotensin-converting enzyme inhibitor enalapril, is followed by regress of LV hypertrophy, accompanied by improvement of the diastolic filling of heart ventricles. The diagnostic value of the dynamic study of the diastolic filling of cardiac ventricles in order to evaluate the effectiveness of long-term antihypertensive treatment, was proved.
