Antithypertensive effectivity of prolonged forms of dihydropiridine calcium antagonists in comparison with the erythrocyte membrane and NI--system status .-Manuscript. The thesis is devoted to a study of NO for the purpose to establish the possible mechanisms of vasodilatatory action of calcium antagonists and to optimise the approach concerning the selection if effective antihypertensive treatment. 98 patiens with essential hypertension EH and 17 healthy subjects were examined. The drastic defficiency of nitric-oxide stable metabolite -nitrit-anion in whole blood and its plasma was found. Simultaneously the level of NO2 in erythrocytes increased.As a consequense the ratio of the level of NO2 in plasma/ the level of NO2 in erythrocytes dramatically diminished having the important diagnostic value. The prominent fall of saturated fatty acid quantity in erythrocyte membranes observed. At the same time levels of some n-6 polyunsaturated fatty acids as arachidonic and docosatrienoic acids were enhanced. For the fist time it was reported that brief and prolonged forms of dihydropiridine calcium antagonist are potent modulator of nitric oxide. Administration of Nifedipine, Corinfar Retard and Amlodipine lead to significant increase of NO2 level in the whole blood and its plasma and to decrease in red blood cells and to improvement of cellular membrane lipid profiel. The adminii stration of amlodipine cause in less deoree of side effects. Thus the performed work demonstrated that administration of brief and prolonged dihydropiridine calcium antagonists lead to normalisation of nitrit -anion in blood and to improvement of cellular membrane lipid profile.
